USP Part D model formulary guidelines
Executive Summary
U.S. Pharmacopeia plans to release a draft version of its Medicare Part D model formulary guidelines 4.0 for the 2009 benefit year on Nov. 6, 2007. Comments on the draft will be accepted until Dec. 31, 2007, and a final version is scheduled to be forwarded to CMS by Feb. 4, 2008. USP's model guidelines expert committee is scheduled to consider the 4.0 version in a closed meeting scheduled for Oct. 1. Comments on the guidelines and requests for revision are accepted by USP throughout the year. The guidelines are used by CMS as a framework for reviewing formulary submissions by Part D plans...
