Vial and box labels for fosphenytoin are being revised following misinterpretations that caused three overdose deaths. Labels reading "50 mg PE/mL," where "PE" means phenytoin (Dilantin) equivalent, have been misread by some as if the vials contained no more than 50 mg phenytoin equivalent. New labels will read "100 mg PE in 2 mL" and "500 mg PE in 10 mL," according to a "Dear Doctor" letter
