Four out of more than 2,000 Medco plans have adopted an incentive-based or closed formulary, a Nov. 9 General Accounting Office report on pharmaceutical benefit managers states. Of approximately 90 formularies managed by SmithKline Beecham-owned Diversified Pharmaceutical Services, one-third are incentive-based or closed.
